Fraud Alert

The Alabama Department of Public Health is warning clients that their personal information may have been compromised.  In a news release today, the ADPH says that they were recently notified by the U.S. Attorney’s Office that an investigation in a currently active fraud case has revealed that the defendants may have stolen names, dates of birth and social security numbers that were on file with the department.  ADPH officials say that notification letters are being sent to clients who may have been impacted by the theft.  
 If you believe that you may have been a victim of this crime, you must submit a special form and supporting documentation to the Department of Justice by July 31.  For more information on this alleged fraud, call 855-292-3496 or visit   www.justice.gov.
